Position Type: Full-Time

Salary: $80,000 to $82,000 plus superannuation per annum

Location: Blacktown NSW 2148

Our client is a Sydney-based, NDIS-registered disability support provider delivering Supported Independent Living (SIL), community access, and in-home support services to participants across Greater Sydney. They support participants to live safely and independently in group homes and individual residences, working closely with families, guardians, allied health professionals, and the National Disability Insurance Agency (NDIA) to deliver person-centred, high-quality care.

The Residential Care Officer is responsible for assessing the individual needs of participants living in Supported Independent Living (SIL) accommodation, and for planning, developing, coordinating, and monitoring the support and training programs that are delivered to them. The role involves working directly with participants, families, and support teams to identify goals and difficulties, coordinating referrals to relevant external agencies and allied health services, monitoring and reporting on participant progress against NDIS plans, and liaising with community organisations, government bodies (including the NDIA), and other service providers to ensure participants receive coordinated and appropriate support. The role also contributes to the preparation of participant reports, funding submissions, and program documentation.

Key Duties and Responsibilities:

• Assess the individual needs of participants in SIL accommodation and develop, implement, and review individualised support and training plans in line with each participant’s NDIS plan and goals.
• Interview participants, families, and guardians to identify the nature and extent of support needs, difficulties, and risks.
• Monitor and report on the progress of participants against their support plans and NDIS goals, escalating concerns to the Care Coordinator or COO as required.
• Refer participants to relevant external agencies, allied health professionals, and community resources that can provide additional support.
• Liaise with community groups, welfare and disability service agencies, the NDIA, and other government bodies regarding participant needs, funding, and service coordination.
• Support participants with daily living activities, community access, and skill development in line with their individualised plans, promoting choice, dignity, and independence.
• Assess community resources and services available to participants and promote awareness of relevant supports among participants and their families.
• Prepare case notes, incident reports, progress reports, and funding or NDIS plan review submissions as required.
• Work collaboratively with the rostering, quality and compliance, and care coordination teams to ensure continuity and quality of support delivery.
• Ensure all support is delivered in accordance with the NDIS Practice Standards, company policies and procedures, and relevant work health and safety requirements.
• Maintain accurate and up-to-date participant documentation in systems (including Brevity and Centro QMS).
• Participate in team meetings, training, and professional development relevant to the role.

Requirements:

• Relevant qualification at AQF Diploma level or higher in disability, community services, health and wellbeing, youth work, or a related field (or equivalent relevant experience).
• Demonstrated experience assessing client needs and planning, coordinating, and monitoring support or training programs, ideally within disability, residential care, or community services.
• Sound understanding of the NDIS, NDIS Practice Standards, and person-centred models of support.
• Strong communication, interpersonal, and report-writing skills, with the ability to liaise effectively with participants, families, and external agencies.
• Ability to work independently and as part of a multidisciplinary team, exercising sound judgement and initiative.
• Current NDIS Worker Screening Check (NDISWC), Working with Children Check (WWCC), First Aid and CPR certification (or willingness to obtain).
• Current Australian driver’s licence and willingness to travel between participant residences.
• Satisfactory National Police Check.
